The Seasons family consists of Seasons Healthcare Management, Seasons Hospice & Palliative Care, Seasons Hospice Foundation, and Seasons Medical Group.


Seasons Healthcare Management (SHM) is headquartered in Rosemont, IL, and employs financial, human resource, Information Technology, and executive staff to provide support to Seasons Hospice & Palliative Care, Seasons Hospice Foundation, and Seasons Medical Group operations.


Seasons Hospice & Palliative Care (SHPC) is a community-based organization with a mission to honor life and offer hope as we find creative ways to add quality to end-of-life care. Founded in 1997, SHPC is one of the largest hospice providers in the nation, providing more than 1 million days of patient care annually. We provide high quality clinical and compassionate care to patients and families throughout the country in our Medicare-certified home care clinical operations and Hospice Inpatient Centers.


Seasons Hospice Foundation (SHF) was founded in 2011 and provides millions of dollars in charity care each year. Its purpose is making memories by fulfilling a wish or providing financial support for a patient/family that will create a lasting memory. The Foundation works in partnership with Seasons Hospice & Palliative Care, and oversees the disbursement of funds that are donated, bequeathed, or acquired through fund raising events for the Foundation. SHF has IRS 501(c) (3) non-profit status.


Founded in 2011, Seasons Medical Group is a team of physicians passionate about making a difference in the lives of their patients. They provide care in a variety of settings including home and facility-based hospice, home and facility based palliative care, inpatient hospice, provide hospital-based palliative care consultation, and, in some areas, offer outpatient palliative care clinics.
